An admission referral form is available on this site in Adobe Acrobat format. A full admission application can be mailed to you. Along with the application, Gracedale requests financial, medical, and social information. An appointment and facility tour with an admission social worker will be arranged following the collection of all necessary information. The applicant's name will then be placed on our waiting list if an appropriate bed is not immediately available. If an applicant is in the hospital and eligible for Medicare, the process can be shortened. Please contact the Social Services Department at the number listed below with any questions you may have.
